PURPOSE:To obtain a laser beam with single wavelength (single mode) by using a phase converting hologram for partially shifting the phase of the wave surface of one interference light on an exposure film. CONSTITUTION:Parallel light beams are made incident upon a phase shifting plate 4 on which a partial phase film 42 is already set up, and a wave surface J' obtained at the time of the wave surface J of light K whose phase is partially shifted by lambda/2, i.e., the wave surface of the light having a phase distortion C due to diffraction, is stored on the hologram 8 based on the light K and reference light I. At time time of the reference light D is irradiated to the hologram 8 storing the J', its reproduced light G is outputted as light having a wave surface with a phase deviation at first, but in accordance with the approach of the light to the position of the phase shifting plate 4, the wave surface is made similar to the wave surface J of the light K whose phase is accurately and partially shifted by lambda/4. The reproduced light G formed shown above is irradiated to a regist layer 5 arranged on the same position as the phase shifting plate 4 and projecting parts are formed by interference stripes obtained by both the light beams F, G.
