PURPOSE:To always read distinctly an image of an original without generating an out-of-focus image, even if the titled device is made small in size and thin, by forming as one body a lens system and a read sensor, and also, providing a means for following the surface of the original at an interval of a prescribed distance. CONSTITUTION:An original 12 which is carried through a carrying path consisting of the upper guide plate 13 and the lower guide plate 14 is pressed against the lower face of a transparent glass plate 8 having a uniform thickness by a rotary roller 9 and carried so that the best focus place can be maintained. A light source 41 for irradiating the original surface, a lens 42 and a read sensor 43 are coupled as one body, and loaded on a carriage 44 together with a recording head 20. The carriage 44 is provided so as to be freely slidable on a rail 45 which has been erected in parallel to the transparent glass plate 8, supported on the transparent glass plate 8 through a roller 16 and maintains a distance to the original surface so as to be constant.
