PURPOSE:To easily form and transmit optional originating information by previously reading out the originating source information written in an original, storing the read information in a storage circuit, and at the time of transmission, outputting the originating source information stored in the storage circuit. CONSTITUTION:At the formation of the originating source information, an operator selects an originating source registering function by a switch or the like in a selecting circuit 3 and a reading circuit 1 reads out an original writing the originating source information. Namely, the selecting circuit 3 transfers a picture information signal obtained by photoelectrically converting the range of the original previously determined by the reading circuit 1 to the storage circuit 2 to store the signal as the originating source information. The output of the reading circuit 1 is simultaneously transferred to a recording circuit 2 to confirm the stored originating source information. At the time of transmission, the selecting circuit 3 transfers the contents, i.e. the originating source information, of the storage circuit 4 to a MODEM 5, and after completing the transfer, transmits the picture information signal of the transmitting original outputted from the reading circuit 1 to the MODEM 5.
