The instrument possesses an attachment which is easy to exchange in that all important functions, for example determination of the movement end-points of the cutting element or the actuation rod (11) and resetting thereof into the position of rest, are realised, so that it is largely independent of the handle. Exchangeability is facilitated inter alia in that the movable handle rests freely against the rear end (12) of the actuation rod (11) and is actuated counter to the force of the restoring spring (14) present in the attachment. According to a further feature of the invention, the movement end-points can be adjusted by adjusting the actuation bolts (12) and a stop ring (13) screwed on to the actuation rods. <IMAGE>
