PURPOSE:To easily and speedily control operation with high precision by positioning stocked sheets vertically by an elevating means and then determining a sending-out position in a front-rear direction. CONSTITUTION:An multi-item input device which sets plural sheets of format sheet recording information consists of a sheet holding frame 1, the elevating means 2, a frame 3, an pushing-out means 4, a sending-out means 5, and a stopper mechanism 6. This sheet holding frame 1 holds a plane rectangular sheet in an expanded or contracted state and mounted on the pedestal 24 of the elevating means 2 in a stack state, and the frame is elevated and positioned freely along an upright frame 21 through both-directional rotation of a motor 26. Then, the elevating means 2 operates to select a holding frame 1 of a sheet to be sent out onto a keyboard and the frame 1 which is already positioned vertically is pushed out forward by the pushing-out means 4. This holding frame 1 is conveyed onto the keyboard by the sending-out means 5 and positioned in the front-rear direction through the operation of the stopper mechanism 6.
