PURPOSE:To attain the simultaneous access to both sides of an optical disk medium having high follow-up properties to response, by adding a photoelectric converting circuit to the movable part of a straight-driving mechanism and connecting this converting circuit to an optical head on the movable part by a connecting means via a reflecting mirror, etc. CONSTITUTION:Photoelectric converting circuits 81 and 82 including optical systems and needed for record and reproduction are provided at the straight- driving sides of arms 51 and 52 respectively. A straight-driving mechanism 2 receives the driving force in the radius direction of an optical disk medium 1 by an electromagnetic function of a magnetic circuit 10 and has an access in the radius direction with high accuracy a direct-driving guidance mechanism 4 such as a bearing, etc. A condensed beam driving mechanism 61 serves as a driving means which drives an objective lens 611 which condenses the beams for record/reproduction onto a surface 01 toward an optical axis and in the radial direction. A pierced hole 511 is formed at the tip of the arm 51 in the direction vertical to the surface of the means 1. The lens 611 is connected optically to the circuit 81 by a rectangular mirror 91 set on the surface opposite to the arm surface holding the mechanism 61.
