PURPOSE:To facilitate inputs of the additional information and to improve the operability of a picture input device, by reading the picture information on an additional information display part concurrently with the picture information on a form through a reading part and storing both information to a memory with relation secured between them. CONSTITUTION:A form on which the original picture information is drawn is put on a form stage, and a reading part 2 moves toward an arrow head by a reading part driving mechanism part 3. The part 2 contains a lens, a CDC one-dimensional image sensor, etc. A picture input device includes an additional information setting part 5 and an additional information display part 4. When 'A-123', for example, is supplied from the part 5, the 'A-123' is displayed at the part 4. The original picture information 1 and the picture information on the part 4 are read by the part 2. At the same time, the additional information supplied from the part 5 is converted into the code information by an encoder part.
