PURPOSE:To facilitate adjusting the pattern and the optical axis of a semiconductor laser output beam by fixing close a semiconductor laser to a laser supporting part of a lens formed body, which has a beam shaping lens and the laser supporting part and is formed as one body, coaxially with respect to the lens. CONSTITUTION:A semiconductor laser 1 is fixed close to a laser supporting part 2b of a lens formed body 2, which has a beam shaping lens 2a and the laser supporting part 2b and is formed as one body, coaxially with respect to the lens 2a.
