A reciprocating internal combustion engine has a head block (6) axially slidably mounted in a bore (5c) formed in a cylinder block (5) in alignment with the cylinder bore (5a), and a cam (3) for moving the head block (6) in its bore (5c) in accordance with the position of the piston (7). The crankshaft (29) of the engine is laterally offset from the axis (31) of the cylinder bore (5a) by the length (r) of the crank radius. The cam (3) is preferably so arranged that the head block (6) reaches its lowermost position during the expansion stroke at a crank angle of 90 degrees; the motoring pressure becomes maximum at the crank angle of 90 degrees.
