In order to permit removal of fatty tissue under the skin with a surgical instrument without having to detach a large area of skin from the fatty tissue and to fold it back, it is proposed that an instrument comprise a tubular piece which is connectable with an aspiration hose and which is increasingly flat from the end to be connected to the aspiration hose to the other end, that the edge located at the flattened end be designed as a blade and that an opening with two edges extending transversely to the longitudinal direction of the tubular piece and designed as blades be arranged in the wall of the tubular piece between the end to be connected to the aspiration hose and the flattened end.
