In the instrument a lance (16) extends from a handle, at the tip (17) of which lance a cutting tool is hinged and is pivotable about an axis of rotation on a circular track and has a cutting tip projecting above the lance. In a preferred embodiment the cutting tool comprises two knives (18, 18') which are hinged on the lance tip (17) so as to be pivotable about axes of rotation, running parallel to each other, and which with the points of their blades are pivotable in opposite directions from a common starting point on circular tracks which complement each other to form a continuous arched section. <IMAGE>
