PURPOSE:To select correctly a designated form slide, in a short time at a prescribed positioning point by abutting preliminarily the slide once before the rotation is stopped and crossing the slide by the continuous rotation. CONSTITUTION:In response to the detection of the designated address mark by a detecting part 8, for example, 2a, a rotating stop member 9 is preliminarily abutted to a form slide loading means once before the rotation is stopped, and by the continuous rotation of the loading means, a crossing part 7 is positively faced to the rotating stop member 9 and simultaneously, both are crossed. Thus, when a designated form slide 2a comes to the prescribed positioning point 11, the loading means can be stopped without fail more than the conventional means, and consequently, the designated form slide 2a can be selected without fail and in a shorter time, for example, about 2-3sec than conventionally.
