PURPOSE:To always transmit and receive a questionnaire independently of the direction of the face of a card shaped responding equipment to the questionnaire device by providing a reception and a transmission microstrip antennas to both faces of a printed wiring board. CONSTITUTION:A printed wiring board 44 of a three-layer structure composed of insulation plates 41, 42 clipping a ground face 43 made of a coppoer foil layer is used and an shoring IC mounting part 12 and a battery 12 are mounted on the surface of the insulation plate 41. A reception microstrip antenna 11a and a transmission microstrip antenna 11d utilizing the copper foil are formed toward the lower end and the reception microstrip antenna 11a and the transmission microstrip antenna 11a are formed onto the surface of the insulation plate 42 opposite to them. Thus, the transmission-reception with the questionnaire device is attained at all times independently of the direction of the card shaped responding equipment with respect to the questionnaire device.
