The instrument has a guiding device (12) including an instrument insertion channel. The guiding device is designed in the form of a guide jacket and extends longitudinally in a lengthwise direction. The guiding device utilizes an arthroscopic fluid to guide an implant or another instrument e.g. trephine, positioning maintaining instrument, through the channel into a defective zone of the body of a human or an animal. An independent claim is also included for a method for minimal invasive treatment of a defect, caused due to accident or generation, in a human or animal.
