Acylated carnitine derivs. of formula (I), esp. the l-isomers, for therapeutic use and their salts, are new: (where R is acyl of a 2-30C aliphatic satd. fatty acid having an even no. of C atoms). Acetyl-l-carnitine (II) and other acylated derivs. of l-carmitine are new. (I) and (II) are useful for treating myocardial anoxia, ischaemia, arrhythmic syndromes, cardiac insufficiencies, and when the energy demand at cardiac level is raised as a result of fatigue.
