PROBLEM TO BE SOLVED: To facilitate wiring and assembly after a movable member is formed and to miniaturize the movable member by inserting and forming a wiring member which electrically connects coils and the respective coils in a movable member having plural focusing coils and/or plural tracking coils and supporting an objective lens. SOLUTION: Focusing coils 5a and 5b are wound to a pair of coil bobbins 11a and 12a formed of resin and tracking coils 6a, 6b, 7a and 7b are wound. Then, a pair of coils 11 and 12 are formed. Each end of respective coils is soldered to the end parts of connection plates 8 and 9 and a movable member 2 is inserted/formed with an integrated part as a nest. The objective lens is fitted to the formed movable member and it is fitted to a base member through four flat springs 7. The other ends of the coils are connected to an outer circuit.
