PROBLEM TO BE SOLVED: To obtain a liquid droplet ejection head and an image forming apparatus which can stabilize an ejection characteristic of liquid from nozzles by increasing a damper width and suppressing effects of pressure waves and inertial flow propagated from pressure chambers to a common passage (shared passage). SOLUTION: The shared passage 32 which supplies ink via an ink supply path 42 to each pressure chamber 36 of a first ejector group 62A and a second ejector group 62B is disposed between the first ejector group 62A and the second ejector group 62B. A passage width is expanded. A damper plate 66 formed of a resin is set inside the shared passage 32. A contact width of the ink with the damper plate 66 is widened because the passage width of the shared passage 32 is expanded, and consequently a structure which can efficiently absorb the pressure waves propagated to the ink is constituted. The printing quality is prevented from decreasing due to an inducement of pressure variation or the like to the pressure chambers 36, and changing of the ink ejection characteristic such as an ink ejection speed or a liquid droplet quantity from the nozzles 16.
