An injection appliance for self-injection e.g. of insulin by diabetic patients has an element which can be given a varying overall length. This element can be cocked and released to move the piston against the container with the drug. The cocking and adjustment of the element is done by a manually actuated cap which is moved longitudinally to cock it and turned to change the overall length. Between the cap and a sleeve, a scale is visible through a window, indicating the relative turn of cap and sleeve.
