A separation case of a medical operation knife is provided to improve convenience and safety of separation work by simply inserting a blade into the separation case and separating the blade during every replacement of the blade. A separation case of a medical operation knife includes a blade take-off device and a blade separation device. The blade take-off device is made of a synthetic resin material and includes a case body(31) and a cover(32). The blade take-off device pushes a rear end of a blade(20) and separates the rear end of the blade from a blade support(11) by elastic power of the case body and the cover when pulling the blade support and the blade to be separated between the case body and the cover. The blade separation device allows the blade to remain between the case body and the cover when pulling only a knob unit(10) out after the rear end of the blade is separated from the blade support.
