PURPOSE: To obtain a medicine for treating cataracts, capable of readily permeating ocular cornea, rapidly being hydrolyzed thereafter to isolate N-acetyl-L- cysteine having anti-cataract action and reaching therapeutically effective concentration and capable of externally administering. CONSTITUTION: This medicine for treatment of cataracts, having the above effects comprises a compound of the formula [R is H or R2 CO; R. is a 1-4C straight-chain or branched chain alkyl, a 1-3C dialkylamino or 1-4C alkoxy or phenyl substituted with phenyl; R1 is a 1-3C straight chain or branched chain alkyl or benzyl; C having star symbol has the same three-dimensional structure as L-cysteine], e.g. S-isobutylyl-N-acetyl-L-cysteine-ethyl ester and a carrier. The medicine is preferably prepared in a dosage form of external preparation for ophthalmic use such as instillation, cream, ointment, suspension, gel or ocular insert.
