PROBLEM TO BE SOLVED: To provide an image forming apparatus or an image forming system, which enables a more flexible system construction.SOLUTION: An image forming apparatus (DICOM printer 14) comprises: a reception part (network transmission and reception part 101) for receiving medical image data (DICOM data); a conversion part (DICOM data conversion part 104) for converting the medical image data into a prescribed format; and an output part (control part 100) which has an association table (output destination table 107) for storing input sources and output destinations in an associated manner and outputs the medical image data received by the reception part to an output destination on the basis of the association table.SELECTED DRAWING: Figure 2
