<p>PROBLEM TO BE SOLVED: To provide an order form for injections which can avoid medical malpractice. SOLUTION: This order form 1 for injections possesses the plural indication parts for ordering injections composed of an indication label with a medicine code label 5 for indicating the information to specify the injected medicine and a name label 6 for medicine, the indication label 7 for the patient information to specify the subject patient for the injection, an indication column 8 for the number of the injection to indicate the number of the injected medicine and an indication column 9 for the canceled injection to indicate the returned medicines by cancellation. It possesses a name entry part 10 for entering the patient name, patient code entry part 11 to indicate the proper patient code with by barcodes and a code entry part 12 for the returned medicine which indicates the code indicating the returned medicine by the cancellation of the injection with the barcodes.</p> |
