The patent describes inhibiting aldose reductase activity with tetracyclic spirohydantoins and previously unknown tetracyclic spirohydantoin derivatives which can be used for treating complications which arise from diabetes mellitus and also a process for preparing spiroimidazolidinediones by reacting a mono- or difluorofluorene with an excess of oxygen and a tetraalkyl- or arylalkyl-trialkylammonium hydroxide.
