The invention relates to a total knee endoprosthesis comprising a stem (1) including a foot (2) intended to be anchored in the tibia and a conical head (3) blocked in the bore (4) of a tibial plate (5) bearing on the level upper part of the tibia and receiving an insert piece (6) made of synthetic material which includes articular surfaces (7) on which a condylar piece (8) can slide in rotation via external and internal condyles. The foot (2) of the stem (1) includes a central core around which two posterior fins (10) and one anterior fin (11) are distributed, the latter being narrower than the two former with respect to an arbitrary cross-section. The tibial plate comprises a boss (12) passed through by the bore (4) and two posterior fins (13) which come to face the fins (10). The boss comprises a positioning slot (14) in which a lug (15) engages, which lug extends the wing (11) of the foot (2) of the stem (1). <IMAGE>
