PROBLEM TO BE SOLVED: To improve workability of a panel zone part in an SRC beam form. SOLUTION: The form 2 having U-shaped cross section is horizontally arranged outside a steel beam 1 with an opening side directed upward. Side plates 21a and 22a of the panel zone part 4 of the form 2 are hinged to side edges of side plates 21 and 22 in the central part of the form 2 openably or closably via hinge 10. A bottom plate 23a of the panel zone part 4 of the form 2 is hinged to the lower edge of the side plate 21a of the panel zone part 4 openably or closably via hinge 9.
