PROBLEM TO BE SOLVED: To provide a forming mold for fabricating a plastic sheet which has fabricating precision and finishing precision enabling formation of a fine optical pattern, is inexpensive, is highly productive in manufacturing an optical article such as a microlens and the like, and is used for forming an optical pattern of a plastic sheet specially for a backward projection type display unit, and to provide its manufacturing method. SOLUTION: The forming mold has a fabricating layer of a metal or a resin on a flat base plate or a cylindrical base plate. The fabricating layer has a geometric structure of spherical or aspheric recesses continuously repeatedly formed in two-dimensional directions. The average surface roughness Ra of the recess portion is smaller than 1μm. The fabricating layer is made by cutting or router working using a blade corresponding to the shape of the optical pattern or transferring a pattern of master version with one or a plurality of the shapes to the fabricating layer.
