PROBLEM TO BE SOLVED: To provide a delivery slip in which a lower sheet is adhered to an upper sheet having two separated parts formed, which facilitates detachment of the lower sheet from the upper sheet with a region facing a separated part of the lower sheet being adhered to the upper sheet.SOLUTION: In a slit 21, a side portion, which is put along sides of a delivery certificate 12 and a receipt 13 on a slip piece 10 to fold so as to protrude into the slit 21, is partly not parallel to the sides of the delivery certificate 12 and the receipt 13, and the part not parallel to the sides of the delivery certificate 12 and the receipt 13 is formed to gradually recede from the delivery certificate 12 and gradually approach the receipt 13 from one end toward the other end of the part. |
