The technique of the abdominal-anal resection of the rectum comprises inferior-medial laparotomy, mobilization of the sigmoid colon and rectum, the tailoring the graft for descending the colon onto perineum. Rectum is cut over the muscles of pelvic bottom below pararectal fat at the level of inferior ampullary division. At the second stage, the mucosa is separated on the side of perineum along the lower semicircle of anus over mucous-skin transition by 1.5-2 cm upwards with the tunnel to small pelvis generated. Through the tunnel, sigmoid colon is descended onto perineum. The excess of the descended colon is removed on Day 12-14. The posterior cell of rectum and anterior wall of the descended sigmoid colon are dissected fashioning colorectal anastomosis. |
