Provided is an image-forming optical system (1) equipped with: image-forming lenses (2, 3) which form a final image (I) and an intermediate image (II); a wavefront confusion element (8) which is disposed further towards an object (O) than the intermediate image (II) formed by the image-forming lenses (2, 3) and imparts phase modulation that causes spatial distortion of the wavefronts of light from the object (O); and a wavefront recovery element (9) which is disposed in a position sandwiching the intermediate image (II) together with the wavefront confusion element (8) and which imparts, to the wavefronts of light with which the intermediate image (II) is formed, phase modulation that counteracts the spatial distortion imparted by the wavefront confusion element (8). The wavefront confusion element (8) and the wavefront recovery element (9) comprise an optical medium and an optical medium that have different refractive indices. The image-forming optical system (1) is capable of imparting phase modulation as a result of the interface shapes of the optical medium and the optical medium.
