PROBLEM TO BE SOLVED: To provide compositions, methods and systems for pulmonary delivery of long-acting muscarinic antagonists (LAMA) and long-acting &bgr;adrenergic receptor agonists (LABA) via a metered dose inhaler (MDI).SOLUTION: Provided is a pharmaceutical composition comprising a suspension medium, a plurality of active agent particles selected from LAMA and LABA, and suspending particles, where the active agent particles and suspending particles form a co-suspension in the suspension medium. Also provided is a pharmaceutical composition where the active agent included in the active agent particle is a LAMA active agent selected from glycopyrrorate, dexipirronium, tiotropium, trospium, aclidinium, darotropium, and any pharmaceutically acceptable salts, esters, isomers or solvates thereof, in particular, glycopyrrorate.SELECTED DRAWING: Figure 8
