<p>PROBLEM TO BE SOLVED: To provide a slip with a sealed letter form where personal information can be displayed in a state that the personal information is not recognizable externally.SOLUTION: A first sheet-like base 2 is partitioned by folding perforations 4a and 4b, and cutting perforations 5b are formed at an equal interval from the folding perforations 4a. Cutting perforations 6a and 6b are formed along an upper edge and a lower edge of the first sheet-like base, and a folding paper part 2c and an intermediate paper part 2b have almost the same width size, and the folding paper part has width size which is superposed over a region located in a right side rather than the cutting perforations 5b of the intermediate paper part. An adhesive layer region is formed in a center, on an upper surface of a second sheet-like base adhesion part 2a, and a pressure-sensitive adhesive layer region is formed in its external region. A releasing agent layer is formed in a region which is superposed over the pressure-sensitive adhesive layer region on a back face of a second sheet-like base 3. The second sheet-like base has a cut 10 formed between the adhesive layer region and the pressure-sensitive adhesive layer region, and only a part where the releasing agent layer is formed of the second sheet-like base, can be released from a pressure-sensitive adhesive layer.</p> |
