[Problem] To provide a medical observation device which is able to ensure visibility for a user and is easier to operate and a medical observation system. [Solution] This medical observation device is provided with: a tube-shaped section; an image-capturing unit that is disposed in a hollow portion of the tube-shaped section and has an optical system for condensing light from an observation object for imaging and an image-capturing element for converting the light condensed by the optical system into electricity to output a captured image signal; a support section that has at least one set consisting of two arm parts and a joint part for rotatably connecting one of the two arm parts to the other of the arm parts, wherein the leading end of the support section supports the tube-shaped section and the image-capturing unit in a rotatable manner about an axis in the height direction of the tube-shaped section; and an arm operation part that is disposed on a side surface of the tube-shaped section that would correspond to the top of the image based on the captured image signal, wherein the arm operation part receives an operation input for permitting rotation of the arm parts.
