<P>PROBLEM TO BE SOLVED: To provide a simple authentication reservation system and method by a QR code reader-equipped Internet connectable portable terminal, using an individual QR code that allows a customer to use it simply. <P>SOLUTION: In this simple authentication reservation system, the individual QR code 1 recorded in a consultation ticket, a member card or the like is read by the use of a camera function of the QR code reader-equipped Internet connectable portable terminal 2 of a patient or the customer, inquiry is performed on a management/storage server 3, on the basis of a URL and a user ID recorded inside the individual QR code 1, and an individual reservation/confirmation site 4 in each the patient or the customer is displayed. The individual QR code 1 is produced for each consultation ticket number or each member card number of each dedicated service organization, and the user ID is allocated with the consultation ticket number or the member card number, and hospital or store information, or the like.
