receiving at a point of care, at a processor of a mobile device, electronic data representative of an image of a printed document; applying the electronic data to an optical character recognition program configured to convert the image of the printed document into text; parsing the text into a plurality of fields, the plurality of fields comprising data representative of information related to the patient's visit to a medical provider; storing the parsed text in a database; anddisplaying at least some of the text on a display screen of the mobile device,
wherein a presentation format of the printed document differs from a presentation format of at least one of the plurality of printed documents.
