PROBLEM TO BE SOLVED: To provide a card formation medium which is supplied to a card issuance device, forms a card to be issued by performing printing based on identification information recorded at a part, and by which relevance between the identification information and a print pattern cannot be decrypted on a user's side.SOLUTION: A card formation medium C1 integrally comprises: a card body part C3 to be an issuance object; and an identification information recording part at least having identification information which is information specifying the content to be printed to the card body part C3, in which the identification information recording part is separated from the card body part C3, and collected in a card issuance device by the time when the card body part C3 to which printing based on the identification information is performed by a printing part is issued as a card after the card body part C3 and the identification information recording part are integrally supplied to the card issuance device.
