The disclosure provides for methods and related compositions for determining the presence or absence of, prognosis, aggressiveness, and progression of prostate cancer. In some embodiments, the methods utilize the differential enzymatic activity in patient samples, for example, of enzymatically active prostate specific antigen (PSA). Also provided are assay platforms (e.g., optical or electrochemical) that can be used for any one of the methods provided.
