The medical instrument (10) has an outer shaft (12) which is provided with window (16) having cutting edge, in the region of distal end. A hollow inner shaft (32) received in outer shaft is provided with apertures at distal end, around longitudinal axis in the region of the window. The apertures are provided with cutting edges cooperated with the cutting edges of window. The hollow inner shaft is connected with low pressure sources (44). An oval window (50) surrounded by a curette (52) is provided in the distal end of outer shaft.
