PROBLEM TO BE SOLVED: To provide a mold form which is used for additionally placing concrete by making a screw part of a tip of an anchor bolt protrude by a preset length, from which a mortar component of the concrete does not flow, and which can be properly supported by a timbering.SOLUTION: A mold form 10 is used for additionally placing concrete to a side face of a lower structure 1 of a bridge, and forming an additionally-placed part 3 from which a screw part 2a of an anchor blot 2 embedded in the lower structure 1 protrude. The mold form 10 comprises a resin foam material 11 arranged with an interval from the side face of the lower structure 1 and composed of a resin foam, and an external face material 12 arranged outside the resin foam material 11 and supported by a timbering 21. In the resin foam material 11, a penetration hole 11a having a diameter smaller than a diameter of the anchor bolt 2 is formed while being aligned in a position of the anchor bolt 2, and a preset length portion from a tip of the anchor bolt 2 is arranged in the penetration hole 11a. |
