PROBLEM TO BE SOLVED: To enable a person to easily carry a card indicating the person's intention and a presenting information sheet bearing presenting information while concealing the person's intention, without complicating a manufacturing process. SOLUTION: A health insurance card 20 with an organ donation intention 21a written on its one surface is pasted separably in a health insurance card pasting area 41 of a concealing label 40 so that the organ donation intention 21a is covered. A generic drug use intention sheet 30 bearing intention information 31 is pasted in a generic drug use intention sheet pasting area 42 of the concealing label 40. Hence the health insurance card 20 with the organ donation intention 21a written thereon and the generic drug use intention sheet 30 with the intention information 31 written thereon are integrated together by the concealing label 40 for concealing the organ donation intention 21a.
