PROBLEM TO BE SOLVED: To provide a delivery slip capable of indicating a receiver precisely at a delivery destination even a receiver was not a person specified as a receiver.SOLUTION: The delivery slip comprises:a surface base material 10 and a tack base material 50a adhered through a peel layer 20 and a bond layer 30. The tack base material 50a is stuck to a peel base material 50c through an adhesive layer 50b. The surface base material 10 and the tack base material 50a can be peeled. On the surface base material 10, an entry column K is provided and on a part of the bond layer 30 corresponding to the position of the entry column K, a self-coloring layer 40 is provided. |
