A touch screen structure and a method of forming the structure are provided. The touch screen structure and the method of forming the structure comprise the followings. A sapphire glass including a first electrode lines is formed. An insulating layer including a second electrode lines is formed on the sapphire glass. Subsequently, a touch screen module and a touch display device including the touch screen structure are provided.
