The implant (10) has a coupling element (16) for coupling an upper bearing part (12) to a lower bearing part (14). The coupling element cooperates with a fixed element e.g. guide (52), and is inserted into a space between bone plates such that the plates are arranged between the parts. A counter bearing (42) for the fixed element is provided on the lower part, and supports the fixed element during a movement of the upper part away from the lower part. The upper part comprises a base body (18) and an insert (20), and the parts are made of plastic e.g. polyetherether ketone.
