<P>PROBLEM TO BE SOLVED: To provide an identification system capable of providing a user an efficient operation for making an on-line application requiring individual identification. <P>SOLUTION: When a user enters entry information including individual information in an entry form 400 displayed on a contractor's terminal 102, a reception server 108 generates a two-dimensional bar code including a reception number for identifying the application and a mail address of the reception server 108. After the user transmits the information to the contractor terminal 102, the user takes a picture of the two-dimensional bar code displayed on the contractor terminal 102 using a portable terminal 106 for analysis. Mail data 902 is generated by using the reception number, the mail address of the reception server 108 and the image data obtained by taking a sheet of identification paper using the portable terminal 106 and transmits the data to the reception server 108.
