PROBLEM TO BE SOLVED: To prepare a questionnaire suitable for a maintenance service provided to a customer.

SOLUTION: A questionnaire device 1 includes: a storage part 12 for storing work report data indicating each report content about a maintenance work performed by a maintenance worker to a product of a customer, questionnaire items composing questionnaires to customers and selection conditions for the questionnaire items; an item selection part 152 for reading from the storage part questionnaire items matching a selection condition which responds to a report content indicated by a piece of work report data stored in the storage part 12 and determining whether the life of a product will end soon on the basis of a use state when the report content shows the use state of the product, and reading from the storage part 12 questionnaire items matching a section condition which responds to the determination result; a questionnaire preparation part 154 for preparing questionnaire data including the questionnaire items selected by the item selection part 152; and a questionnaire data output part 16 for outputting the prepared questionnaire data to a customer terminal device 4 or a work report terminal device 2.
