PROBLEM TO BE SOLVED: To prevent a document with a code image containing conflicting contents from being created in a digital composite machine which comes with a function to prevent unauthorized copying.

SOLUTION: An image forming apparatus comprises: reading means for reading a document to acquire a document image; detection means for detecting a code image containing output control information from the document image acquired by the reading means; detection setting means for setting whether to detect the code image by the detection means; additional setting means for setting whether to add the code image to the acquired document image; operation restriction setting means for setting operation restrictions in operating mode for non-detection when non-detection of the code image is set in the detection setting means; and output means for outputting the acquired document image based on output contents corresponding to a user's instruction.
