The present invention relates to iron oxide-based nanoparticles comprising: an iron oxide-based central particle; a shell coated with a water-soluble material selected from the group consisting of ATP, NAD, ADP, AMP and NADP which are non-polymerized; and gluconic acid securing binding amounts and binding conditions to ensure the optimum biocompatibility on the surface of the shell, a preparation method thereof, and a contrast medium and a composition for diagnosis and treatment of cancer, containing the same. The iron oxide-based nanoparticles surface-modified with gluconic acid of the present invention reduces a degree removed through uptake by the reticuloendothelial system, thereby improving delivery to a target organ or lesion, and can be used as a contrast medium for diagnosis and treatment capable of simultaneous imaging and treatment.
