PROBLEM TO BE SOLVED: To provide a facsimile delivery confirmation device and a delivery confirmation program which reliably inform a receiver of the receipt of FAX information and can reduce procedures of the receiver necessary for a notice of delivery confirmation to a sender as compared to the case that the present construction is not provided. SOLUTION: A delivery confirmation server device 2 has: a FAX information accepting means 200 which accepts received FAX information 213 including information on the sender; an electronic mail transmitting and receiving means 202 which transmits forward mail to which the received FAX information 213 is added to a destination previously associated with the information on the sender; a return FAX generating means 203 which generates return FAX information 214 according to a content of delivery-confirmed mail when receiving it from the destination associated with the information on the sender; and a return FAX information output means 204 which transmits the return FAX information 214 generated by the return FAX generating means 203 to the sender.
