PROBLEM TO BE SOLVED: To provide a flat die for form rolling capable of making groove depths of a workpiece uniform without changing the total length of a die or the time for machining the workpiece. SOLUTION: The flat die for form rolling includes a lead-in part 11, an adjusting part 12, a finishing part 13, and a relief part 14. The finishing part 13 comprises a first parallel finishing part 13a, a finishing relief part 13b, and a second parallel finishing part 13c. The finishing relief part 13b is disposed between the first parallel finishing part 13a and the second parallel finishing part 13c. Further, the gradients of a tooth top line 17, a pitch line 16, and a tooth bottom line 15 of a die tooth in the finishing relief part 13b are set in the directions opposite to the directions of the gradients of the tooth top lines 17 of die teeth in the lead-in part 11 and the adjusting part 12.
