PROBLEM TO BE SOLVED: To provide a form disassembling device by which a panel plate and a frame material can be accurately and efficiently taken apart from each other. SOLUTION: The form disassembling device X includes a plurality of holding mechanisms (a horizontal-bar holding mechanism A, a first outer-vertical-bar holding mechanism B, a second outer-vertical-bar holding mechanism C and an intermediate-vertical-bar holding mechanism D) for respectively holding a horizontal bar W1 and vertical bars W2, W3 of the form K from the thickness direction, and a pushing-up mechanism E for pushing up the panel plate P of the form K in the direction of moving apart from the frame material W. The form disassembling device X separates the frame material W and the panel plate P by pushing up the panel plate P by the pushing-up mechanism E to release the fixed state by a fixing implement N in the state where the bars are respectively held by the holding mechanisms from the thickness direction.
