The instrument (10) has a hand grip (40) arranged at a main part (20). The hand grip comprises a fixed handle (42), and another handle (44) i.e. traction lever, that actuates a movable part (30). The movable part is connected with a perforation part (50) i.e. perforation needle, in an undetachable manner. The perforation part is merged with another perforation part (22) for perforation process. The former perforation part is guided commonly with the movable part to the main part. A releasing pin (48) is provided for releasing the main part and the movable part from the hand grip.
